To schedule an appointment, please call the office at (702) 734-9600. When you come in for your eye exam, please bring a list of your current medications, glasses (and the prescription if you have it), contact lenses (and the prescription if you have it) or contact lens boxes (have prescription on it) and any solutions or drops you use for your eyes. If you are covered by medical insurance and/or a vision plan, please remember to bring your insurance cards for both and your photo identification card. Your eyes may be dilated during the exam. If you are uncomfortable driving due to this procedure, you may want to bring your sunglasses and/or a driver to bring you home. We will also have disposable sunglasses for those diated patients who may not have sunglasses. Please fill out and bring our Appointment Intake Forms (click here to access forms) with you when you come in for your eye exam to save time. Thank you.
